Book the Focused Movement Care session (25min) even if it is your first visit. Be sure to fill out the forms prior to your appointment.
If you see the option “Contact to Book” you can choose to stop by in the available time to be seen or text 435-559-9354 to reserve the time slot. You will still need to create a profile and fill out the necessary forms before the appointment time.
